Older adults, adults with disabilities, family caregivers, and the case managers coordinating their care.

Older adults aging in place

Most older adults we serve are not deciding between home and a facility. They have already decided: they are staying. The question is what has to be true for that to keep working. Usually it is some combination of the household staying manageable, the isolation being interrupted, and the overnight hours being covered.

The services that do most of that work are homemaker support, adult companion services, and night supervision.

Adults with disabilities

For adults with disabilities the goal is often the opposite direction: not maintaining a situation but expanding it. More independence, more community access, more skills held personally rather than borrowed from staff.

Individualized home supports, individual community living support, and semi-independent living skills are built for that, with support deliberately fading as capability grows.

Family caregivers

Family caregivers are the largest and least supported group in home care. They are usually doing the majority of the work, often with no training, frequently while holding a job, and typically without a single scheduled day off.

We support them two ways: respite care that produces actual rest, and family training that makes the hours they are covering safer and less exhausting. Caregiver collapse is one of the most common reasons a home care arrangement ends, and it is largely preventable.

Case managers and care coordinators

For case managers, what matters is whether a provider does what they said. We aim to be the kind of provider that answers the phone, reports changes without being chased, documents accurately, and declines a referral we cannot staff instead of accepting it and failing quietly.

Eligibility and referrals

Do you work with people who have dementia?
Yes. Night supervision, respite care, and family training are the services most often used in households where someone has dementia, frequently in combination.
Can you support adults with developmental disabilities?
Yes. Individualized home supports, individual community living support, and semi-independent living skills training are all commonly used by adults with developmental disabilities living independently or with family.
Do you accept referrals directly from case managers?
Yes, and a large share of our referrals come that way. Case managers can call (612) 345-4490 or email info@allcaremsp.com to discuss authorization, availability, and timing.
What if I am not sure my family member qualifies for services?
Call and describe the situation. If a waiver program or case manager is involved we can talk through it, and if we are not the right provider we will tell you where to look instead.
