Home and community based services in Minneapolis, MN
Nine distinct support services, delivered to individuals and families throughout the Twin Cities metro. Most plans combine two or three of them.
Two categories of support
Our services fall into two groups. Basic support services provide the hands-on help someone needs to get through the day safely. In-home support services focus on building skills, so the amount of help needed can decrease over time.
Most people use a combination. Someone might receive homemaker support twice a week, respite care monthly so a family caregiver can rest, and skill-building sessions aimed at eventually managing their own medications. One plan, one point of contact.

Four steps from first contact to care in the home.
Call or write
Tell us the situation in plain terms. No forms to start.
Intake conversation
We talk through routines, goals, and what support is actually needed.
Written plan
We document the plan and coordinate with your case manager.
Start and adjust
We begin, then revisit the plan once it meets real life.
Who actually shows up at the door
The service list matters less than the person delivering it. Here is how we handle that.
Background studied
Every staff member completes a Minnesota background study before their first shift.
Oriented per client
Staff are trained on your specific plan before working alone in your home.
Matched deliberately
We match on language, location, and temperament, and we will change a pairing that is not working.
Kept consistent
You get the same people rather than a rotating cast. We manage for this actively.

Not sure which service fits?
That is the most common call we get. Tell us what is happening and we will point you to the right support, even if it is not us.
