How services are paid for
Most of the people we serve use a Minnesota waiver program. Here is how that generally works, and what to do if you are not sure what you qualify for.
Minnesota waiver programs
Most home and community based services in Minnesota are paid through Medical Assistance and its waiver programs. The common ones are the Elderly Waiver (EW), Community Access for Disability Inclusion (CADI), the Brain Injury (BI) waiver, the Developmental Disabilities (DD) waiver, and Alternative Care (AC).
Which services you can receive, and how many hours, is set in an assessment and written into your plan. That is done through your county or lead agency, not through us.
If you already have a case manager
They are the fastest route. They can tell you exactly which services are authorized and how many hours, and they can send us a referral directly. Have them use our referral form.
If you do not have a case manager yet
Start with an assessment through Hennepin County or the Senior LinkAge Line. They determine eligibility and connect you with case management. See our client resource links for where to begin.
Private pay
Services can also be paid privately without going through a waiver. This is often simpler for occasional needs like respite, or for households that do not qualify for a program. Call us and we will talk through what the hours you are considering would actually cost.
What we will tell you honestly
We do not determine eligibility and we will not guess at it to keep you on the phone. If we think you qualify for something we do not provide, or that another route would serve you better, we will say so.
